AppointmentsOur office offers flexible hours to accommodate most patients' schedules. For your convenience, daily and evening hours are avaliable. We will also accommodate patients needing emergency care. Every effort is made to keep waiting time in the office to a minimum. However, emergencies arise, patients arrive late, and some patients need more time than others. If we do fall behind, we hope you will understand. Please remember that you could be the patient who requires extra attention. If you find it inconvenient to wait, we will be glad to reschedule your appointment. Should you need to cancel your appointment, kindly notify our office at least 24 hours in advance (48 hours for surgical appointments) so that other patients can be scheduled during the vacated time slot. here to fill out and print registration forms. PaymentPayment is expected at the time of your visit. Payment in full is the responsibility of the patient. Please keep in mind that patients are responsible for deductibles, co-insurance, co-payments, and services not covered by insurance. Itemized receipts will be given only at the time of the visit. If you require extra copies, please request them at the time of your visit, as no itemized receipts will be prepared at the end of the year. Cash and checks are acceptable forms of payment at our office. Please speak with the office manager regarding other forms of payment. InsuranceOur office participates with most insurance plans. Our receptionists can provide you with a current list of insurance plans in which we participate. If you have health insurance, please bring your insurance information with you to all visits. For all insurance plans, patients are responsible for payment of their deductibles, co-insurance, and co-payments. These payments are required at the time services are rendered. Patients are also responsible for obtaining any necessary referrals prior to their office visits. Patients who do not have a valid referral must pay for their office visit in full or reschedule their appointment.
